Records Requests

To make a request for a police incident or crash report, or another police public record, please fill out the below records request form. If you need to make a more specific inquiry, please email the Records Clerk at recordsclerk@perrytwp.org.

Records and Public Records are defined respectively in ORC §§ 149.011 (g) and 149.43 (A)(1). We pledge to provide prompt inspection of public records and provide copies of public records with a reasonable amount of time. We ensure that any and all redactions within publicly released records are pursuant to the ORC.

  2. Under Ohio law, you’re not required to put your request in writing. You are also not required to identify yourself.  However, we ask you to provide the information below because it will help us expedite your records request.

    We try to respond as promptly as possible to all records requests. Please allow up to 10 business days to reply to your request, and please understand that some requests may take longer to fulfill, based on the amount of records requested or the type of record requested (e.g., video footage, historic maps, etc.)

    If you prefer to make a verbal request, please contact our office at (614) 889-2669, or visit the office at 7125 Sawmill Rd, Dublin, OH between the hours of 8 AM and 4 PM. 

  3. If you do not provide a contact method or a method of delivery, we will be unable to send you the requested records. 

  4. If you request a record by email, an email address must be provided, otherwise we will be unable to send you the requested records.
